What is the Self-Directed Community Benefits Program?
The Self-Directed Community Benefit (SDCB) program empowers members to take control of their care by choosing how their benefits are delivered—and who provides them.
​
Through this program, members work closely with a Care Coordinator (assigned by their MCO) and a trained Support Broker from Visions to create a personalized Care Plan. This plan outlines the goods and services that support their needs at home and in the community, including personal care, therapies, and more.
​
At Visions Case Management, our Support Brokers are experienced, compassionate, and responsive. Your Broker will help you:

Transition smoothly from agency-based services

Create and submit a person-centered care plan

Monitor your budget and maintain program eligibility

Access community resources and therapeutic supports

Stay informed about your rights and options—every step of the way

What Your Support Broker Does
Your Support Broker is your personal guide through the Self-Directed Community Benefit (SDCB) program. They're here to help you take full advantage of your benefits while making sure the care you receive reflects your needs, preferences, and goals.

At Visions, our Support Brokers will:

Help you design a person-centered care plan tailored to your daily needs and goals

Support you in hiring and managing caregivers and service providers

Assist with budget monitoring and help ensure funds are used appropriately

Work closely with your Care Coordinator to keep services aligned and up to date
-
Guide you through the approval and renewal process to maintain eligibility
-
Keep you informed about your rights, resources, and options
-
Be available when questions come up—or when things change unexpectedly

Whether you’re new to self-direction or just need a more reliable advocate, your Visions Support Broker will be with you every step of the way—answering questions, solving problems, and helping you stay in control of your care.
