What is Delegated Care Coordination?

Delegated Care Coordination (DCC) is a specialized program delivered in partnership with Presbyterian Health Plan to support individuals across New Mexico who are enrolled in the Mi Via Waiver or Developmental Disabilities (DD) Waiver.

Through this program, Visions Case Management provides trained Care Coordinators who work directly with members and their interdisciplinary care teams to ensure needs are identified, addressed, and consistently supported.

We focus on a member-centered approach that helps individuals live safely and independently in their communities while supporting the full spectrum of care needs—including:

Medical needs
Behavioral health needs
Social supports
Community-based services and resources

What Your Care Coordinator Does

Your Visions Care Coordinator is your advocate, guide, and go-to support for making sure your health and wellness needs are met—on time, with care, and with your voice at the center of every decision.

Our Delegated Care Coordinators work directly with you, your care team, and community supports to ensure nothing falls through the cracks. Here’s what you can expect:

Personalized Support Services

Schedule and coordinate doctor visits, therapies, and follow-ups
Send reminders to help you keep track of important appointments
Assist with accessing non-emergency medical transportation
Communicate regularly to ensure your care stays on track
Offer rapid responses and consistent check-ins

Care Coordination Across Systems

Create a bridge between healthcare providers, schools, and service agencies
Support providers with coordinating services for chronic or complex health needs
Help manage care in the least restrictive setting to support safety and independence
Provide honest, up-to-date information about your rights, benefits, and next steps

Resource Referrals and Advocacy

Connect you with community resources like support groups, food access, or housing
Advocate for your needs in medical, behavioral, and social care settings
Empower you to make informed decisions about your health and wellness
Help you navigate changes in your care—such as hospital discharges or new diagnoses
